CLAIMS PROCEDURE FOR OVERSEAS TRAVEL

pleasant trip! We invite your attention to the following table, which will help you in the event of any loss, accident or sickness.

Our Service Center is equipped to provide you with the necessary guidance in your situation, and will direct you on claims procedure.

Note: Failure to call this number in respect of Medical Accident & Sickness Claims (Travel) shall invalidate your claim,

 if any. In case of OPD treatment (where hospitalization is not required), please collect all bills and receipts and file for reimbursement with TATA-AIG,

Claims Department, at the address mentioned above. This is a General check-list of documents, please check for availability

of coverage under the policy.

Medical Accident &

Sickness Expenses** (Outside India)

1.        Please contact our Service Center at the number given above and obtain a Claim Form from them and fill in. It is necessary to obtain the attending physician’s signature on the Form.

Medical Sickness Dental Relief **

(if covered under the policy)

SAME AS DOCUMENTS REQUIRED IN MEDICAL ACCIDENT & SICKNESS EXPENSES

1.        Please contact our Service Center at the number given above and obtain a Claim Form from them and fill in. It is necessary to obtain the attending physician’s signature on the Form.

2.        Please collect all bills/receipts/invoices and send all documents to our Corporate Office at the address mentioned above

Loss of

Baggage

1.        Claim form

2.        Property Irregularity Report (obtained from Airline)

3.        Copies of Correspondence with the Airline authorities/Others confirming the loss and details of compensation.

4.        Individual list of items in each baggage with approximate cost of each item.

5.        Copy of the passport/Visa with Entry & exit stamp

6.        A copy of the Travel certificate/ policy

1.        Intimate the airline about your loss and lodge complaint, obtain the PIR Property Irregularity report.

2.        Claim Form can be obtained from our Service Center.

3.        Fill in the Claim Form and send all documents to our Corporate Office at the address given above.

Delay of

Baggage

1.        Claim form

2.        Property Irregularity Report (obtained from Airline)

3.        Original bills/receipts/invoices pertaining to expenses   incurred/purchases made towards necessary personal effects, during the delay period

4.        Copies of Correspondence with the Airline authorities/Others and details of compensation.

5.        Copy of the passport/Visa with Entry & exit stamp

6.        A copy of the Travel certificate/ policy

1.        Obtain confirmation of the delay from the airline

2.        Claim Form can be obtained from our Service Center.

3.        Fill in the Claim Form and send all documents to our Corporate Office at the address given above.

 

NOTE: Baggage delay in the Republic of India is not covered

Loss of

Passport

1.        Claim form

2.        Copy of new passport & previous passport (if available)

3.        Original bills/invoices of expenses incurred for obtaining a new passport

4.        Copy of FIR/ Police Report

1.        File a complaint with the local police & Contact with the Indian Embassy, where ever necessary

2.        Submit all documents to our Corporate office at the address given above, along with a detailed statement.

Trip Delay

1.        Claim Form

2.        Copies of Correspondence with the Airline authorities certifying about the cause of delay.

3.        Original Bills of purchases made/ expenses incurred during the period of delay for meals and lodging covered as the result of a covered hazard.

4.        Copy of Ticket & Boarding Pass

5.        Copy of passport/Visa with Entry & exit stamp

6.        A copy of the Travel certificate/ policy

1.        Claim Form can be obtained from our Service Center.

2.        Fill in the Claim Form and send all documents to our Corporate Office at the address given above.

 

NOTE: Coverage under trip delay is for covered hazards such as, Equipment Failure, Inclement Weather and Strike, to be confirmed by the airline.

Trip Cancellation

1.        Claim Form

2.        Detailed statement explaining the circumstances resulting in Trip Cancellation.

3.        Copy of the Death Certificate, incase of death / Medical reports specifying the nature and diagnoses of injury/sickness of insured, Travel companion, immediate family member or travel companion (s) immediate family member.

4.        Copy of Ticket & Boarding Pass for yourself and your companion (s)

5.        Receipts towards for pre-paid travel arrangements i.e ticket cancellation/hotel bookings etc. which are non-refundable in nature.

6.        Copy of passport/Visa with Entry & exit stamp

1.        Claim Form can be obtained from our Service Center.

2.        Fill in the Claim Form and send all documents to our Corporate Office at the address given above.

Trip Curtailment

1.        Claim form

2.        Copy of Ticket & Boarding Pass for yourself and your companion (s)

3.        Copy of the Death Certificate, incase of death / Original admission/discharge card together with complete medical reports specifying the nature and diagnoses of your immediate family member.

4.        Receipts towards pre paid travel arrangements i.e ticket cancellation/hotel bookings etc, that are non-refundable in nature.

5.        Copy of passport/Visa with Entry & exit stamp

1.        Claim Form can be obtained from our Service Center.

2.        Fill in the Claim Form and send all documents to our Corporate Office at the address given above.

Personal Liability

1.        Full statement of the facts in writing along with Witness statements

2.        Any other documents relevant to the incident, including Summons, Legal Notice etc.

3.        Any other information you would like to share with us.

1.        Inform our Corporate Claims Dept. immediately (at the address given above) giving full details of the incident.

2.        Do not commit any benefit/compensation or enter into any agreement.

Hijack

1.        Claim Form

2.        Full statement of the events in writing

3.        Airline correspondence (copy of Passenger List etc.)

4.        Copy of ticket/ Boarding Pass

1.        Claim Form can be obtained from our Service Center.

2.        Submit all documents to our Corporate office at the address given above.

Bail Bond

1.        Full statement of the events in Writing along with Witness Statement

2.        All relevant legal documents and proof of bail amount

2.        Fax details to the Service Centre/Corporate office.

Home Burglary

1.        Claim form

2.        Police report - FIR & final Investigation report

3.        List/value of items stolen

1.        Arrange to protect the point of entry of the Burglar

2.        Intimate the police and Corporate office and fax a detailed statement of the incident to the Corporate office, at the address given above.

Compassionate visit/Study Interruption/

Sponsor Protection

(if covered under the Student Guard Policy)

1.        Claim form

2.        Detailed statement explaining circumstances necessitating your visit, including complete details of your itinerary duly supported by flight tickets and boarding pass and details of payments made to the airline.

3.        Original admission/discharge card together with complete medical reports specifying the nature and diagnoses of your immediate family member.

4.        Copy of students ID card, full details (semester wise) of the course being undertaken, number of semesters completed/remaining.

5.        Copy of admission letter / University brochures/ prospectus, stating the total amount payable to the institute for the course taken.

6.        Copy of passport/ Visa with entry /exit stamp of yourself ( in case of illness of a relative in India) or your family member(In case of your illness abroad)  

7.        Copy of the Death Certificate, incase of death.

8.        Copy of your ration card and proof of identity of your family member (copy of driving license, passport, election card etc)

9.        Copy of your travel insurance policy.

2.        Submit all documents to our Corporate office at the address given above.

Accidental Death &

Dismemberment

Tata AIG Claims Department will advise on documentation, upon receipt of claim notification.

1.        Claim Form can be obtained from our Service Center.

2.        Collect all documents pertaining to the loss including correspondence with Carrier and send to our Corporate Office at the address given above.

* Note:  We may call for additional documents/ information as relevant.

** If any hospital does not submit a bill to you for the treatment/service rendered, please intimate our Service Center before you leave the hospital.
